A parent or guardian is required to accompany their minor on their first visit. For subsequent visits, a minor consent can be completed to allow for them to be seen without a parent or guardian.

Co-payments must be rendered at the time of the visit. We accept Visa, Mastercard, American Express, debit cards, cash and checks.
